Upper Room Christian School graduates from the Class of 2020 and earlier may request a copy of their transcript by submission of the following.

 

1. There is a $20.00 processing fee.  Payment may be done by cash, check or credit card.  All checks must be made out to Upper Room Christian School.

2. Fill out the Alumni transcript request form and have it faxed/mailed or handed it.

 

 

When making payment over the phone by credit card, contact the main office at 631-242-5359 EXT 201.

***Your request will not be accepted unless both payment and form are handed in.***

 

Once the Guidance Department receives everything, it will take a total of 48 hours to send out the transcript through 2 day Priority Mail.
